Evening and Weekend Appointments Available Now

You can now access additional Primary Care Services in the evenings and at weekends.

Improved Access is a Wiltshire-wide initiative where local practices are working together to offer increased availability of services and appointments in the evenings and the weekend. These appointments are available 18:30-20:00 Monday-Friday, and some Saturdays, Sundays and bank holidays.   This collaboration  by  practices  will ensure all patients in Wiltshire  have this option, but this might mean you will be offered an appointment at another practice.

Appointment types will vary - some being face-to-face, some on the phone - and there will be a mixture of bookable and same-day appointments available across the week. We will be offering  appointments with GPs as well as with other healthcare professionals, such as a Practice Nurse or Health Care Assistant . We are keen to balance the importance of continuity of care with convenient access for patients, while ensuring that the service is safe.

To book such an appointment, contact your local Surgery during normal opening times. A receptionist will explain the process, and ensure the booking is appropriate. You will need to give consent to use this service.

For more information, please contact your usual GP surgery.

Request for letters or forms completion

Request for letters or forms to be completed by GP's

If you require a letter from your GP, (for example for your employer, housing office, or Gym) in relation to your health, there will normally be a standard fee of £16.50 as this does not form part of regular NHS work.

As you are aware demand for appointments is very high and our priority must be to see ill patients first.

Requests for the completion of forms or letters is an administrative task and will be completed by the doctors after they have seen their patients and finished all their clinical work.

Please hand forms or requests for letters into reception.

We will try to complete these within 2 weeks but sometimes this may not be possible.

Patients asking GP's to complete forms for things like parachute jumps or taking part in TV shows need to be aware of the following information.

We take in each request on a case by case basis & your GP will need to view the form & your medical records before agreeing to complete it. They will have to consider how the form is worded & if they able to answer all of the questions.

Each form presented to your GP must be accompanied by an attachment which will include guidance notes for the GP. (Please ensure these are brought in with the form as your GP will not be able to complete it without them).

As this is not part of regular NHS work there is a minimum charge of £16.50 (or more if the form is complex) for each form completed.

Please ensure you have completed all your sections of the paperwork & signed the declaration prior to handing the form in to reception.

This information does not apply to life insurance reports or forms for DWP
